Once registered, you will be asked to fill out an intake form and our patient relations department will contact you to schedule a medical intake

If you have Dutch insurance and wish to visit a specialist, you will need to speak to your GP about getting a referral before booking a specialist appointment
Ensure you are informed as to whether your insurance will pay for your medical care with us, or whether you may need to cover part of the costs

Fill out our intake form. When you register you will be asked to fill out an intake form. This has questions about your general health and medical history. This will inform your doctor on your health needs so that they can provide you with tailored and high quality care. You will then be contacted by our Patient Relations department to schedule a medical intake if necessary.
If you wish to visit a medical specialist at our polyclinic (such as a dermatologist, gynaecologist or ENT specialist), and you have Dutch insurance only, we advise that you first see your GP to get a referral. You may still visit our specialists without a referral, but keep in mind that your Dutch insurance will not cover the costs of the medical care unless you have a referral. A referral must be obtained before the specialist appointment takes place for it to be valid for reimbursement purposes.
For non-Dutch insured patients no referral letter is required.

The Dutch health insurance system is likely to be different than what you are used to.
