DE-REGISTER FROM THE IHCH
Are you leaving the IHCH?
We wish you well. Please de-register from our services by completing our official de-registration form below.
If you are moving to another GP Family practice within the Netherlands, please provide us details of your new GP in the additional info/comment box of the form so that we can arrange an electronic transfer of your medical files.
If you are leaving the Netherlands and would like to request a copy of your medical files, we can provide your complete records on a USB stick. Please click yes from the dropdown menu of the form. We require that you collect it in person from the International Health Centre the Hague.
Please keep the following in mind:
- We require a minimum of two weeks notice in order to complete the de-registration process – an administration fee may apply
- Please remember to bring official photo identification when collecting your medical files
- All medical files must be collected from our location at Jan van Nassaustraat 125, 2596BS The Hague – Please report to Reception on the ground floor for assistance
