The IHCH Diabetes Program focuses on your blood glucose level to help keep it as normal as possible. A stable blood glucose helps prevent diabetes whilst maintaining your quality of life and slowing the progression of any health-related complications.
During the consultations, we discuss and provide you with:
- Information to prevent diabetes and advice about treatment options and related complications
- Offer healthy lifestyle advice and advise about treatment that may include the use of medication and/or insulin therapy
- Train you how to measure your own glucose levels and/or how to inject your insulin, if necessary
- Help make you aware of important healthy diet options and provide referrals to a dietician and/or a nutritionist for advice, if necessary
First appointment- 45 minutes
Follow-up appointments- Every three months for a maximum of 30 minutes
All persons with Dutch insurance wanting consultations within our Diabetes Program, will require a referral from their GP or specialist first. If you do not have a referral letter, it is likely that your Insurance company will not reimburse the fees.
