Should I have an intake appointment?
Please note that having an intake appointment is not obligatory. You are already successfully registered with the departments you selected on the registration form once you receive the confirmation email, which includes the Intake Form as an attachment.
If you wish to have an intake, we currently offer two types of appointments: Medical Intake and Patient Relations Intake.
Medical Intake
This is recommended only for patients with an extensive personal medical history (e.g., those diagnosed with chronic conditions). During the Medical Intake, your medical history will be added to your digital medical file. Please note that this appointment is billed as a GP medical consultation.
The Medical Intake ensures your medical history is properly documented before your first consultation with the GP. However, current health complaints, prescriptions, or medication renewals cannot be addressed during this appointment. For those matters, please book a regular GP appointment.
If you registered multiple family members with our GP practice, the Medical Intake should only be booked for the member(s) with significant medical history.

Patient Relations Intake
For non-medical matters—such as understanding how the GP practice works, insurance questions, or general inquiries—you may choose to book a Patient Relations Intake.
Patient Relations is located at the main entrance desk. You can also reach them by phone by selecting option 6 from the telephone menu, or by emailing your questions to ihchinfo@ezorg.nl.
